Teams desiring to pay by credit card must provide the card holder's name, account number, and expiration date. Applications being paid by credit card may be FAXed to 401-667-4920 for faster service.

All applications will be acknowledged by email to the indicated Point of Contact. If email is not received within 1 week, contact us immediately.
